It can be easily distinguished from other Hipposideros by its larger body size and a distinct white, cream, or yellow flash along the side of each shoulder, which extends as a stripe down the side of the upperparts. It is primarily a cave-roosting bat (Kingston et al., 2006), and has been recorded roosting with Rhinolophus affinis at a cave in Penggerukan, North Sumatra (Whitten et al., 2000).
